Agreements

The Agreements section allows you to view, manage, and access all agreements and contracts associated with your provider account within the Sensible Care platform. This section provides centralised access to all agreements completed during onboarding and throughout your provider relationship with Sensible Care, ensuring that your contractual documents are always available whenever required.

To access this section, click Agreements from the left-hand navigation menu.

How the Agreement Process Works

After your registration is submitted, you will receive two emails. The second email comes from the Compliance Officer and contains a secure SnapForms link to complete your provider agreement.

What You Are Required to Do

When you open the SnapForms link, you must:

  • Review the agreement terms
  • Verify your business information
  • Confirm your service details
  • Complete the digital signature process

Accessing an Incomplete Agreement

If you were unable to complete the agreement through the email link at the time of receiving it, you can navigate to the Agreements section from the left-hand menu at any time to access and complete your pending agreement.

Important: Your provider account cannot be fully activated until the agreement has been signed and approved.

Completing the Service Provider Agreement

The Service Provider Agreement is a formal agreement between you as the Supplier and Sensible Care Pty Ltd for the provision of services under the Home Care Package, NDIS, and CHSP programmes.

Step 1 — Date of Agreement

At the top of the agreement form, enter the date on which you are completing and signing the agreement.

Important

  • Ensure this reflects the actual date of completion
  • Do not use a past or future date

Step 2 — Selecting Your Services

The agreement requires you to select the specific services your organisation will be providing to Sensible Care clients.

  • Mowing
  • Allied Health
  • Cleaning
  • Gutter Cleaning
  • Podiatry
  • Laundry Services
  • Carpet Cleaning
  • Window Cleaning
  • Food Supplier
  • Other

Select all services that are applicable to your organisation before proceeding.

Step 3 — Reviewing Agreement Terms

Before submitting, carefully read through all agreement terms and conditions presented in the form.

The Agreement Outlines Your Obligations Including

  • Service delivery standards
  • Fee structures
  • Compliance requirements
  • Privacy obligations
  • Certification of compliance responsibilities

Ensure you understand and agree to all terms before completing your submission.

Step 4 — Submitting the Agreement

Once you have entered the date, selected your services, and reviewed all terms, you can complete your submission.

Options Available

  • Click Submit to finalise and complete your agreement
  • Click Save and Complete Later if you need additional time to review the document

Viewing Your Agreement Status

The Agreements page displays all agreements associated with your provider account in a structured list format.

What Is Displayed

Each agreement entry shows:

  • Agreement name
  • How it was generated
  • Current status

Master Service Agreement

The Master Service Agreement is the primary agreement completed during onboarding and is auto-generated via SnapForms.

Once the agreement has been successfully signed and submitted, it will display a Signed & Completed status confirming that your contractual obligation has been fulfilled.

Viewing the Agreement Document

To view the full content of a completed agreement, click the View Document button displayed beneath the agreement entry.

  • Review the terms and conditions of your signed agreement
  • Access agreement details at any time for reference or record-keeping purposes
