Care Team

The Care Team tab shows the details of your assigned care team or care manager. From this page you can view contact information, start a phone call, or send an email directly through the portal.

Care Team Details​

Profile​

Displays the following information for your assigned care team member:

  • Care team name
  • Role
  • Current status

Contact Actions​

The Care Team page provides two direct contact options:

Call Care Team​

Click Call Care Team to start a phone call using the listed phone number directly from your device.

Send an Email​

Click Send an Email to open an email addressed to your care team's listed email address.

Contact Details​

The following contact information is displayed on the Care Team page:

  • Available hours
  • Phone number
  • Email address
  • Location

Outside Business Hours​

If you contact your care team outside of their available hours, the portal will advise you to:

  • Leave a voicemail
  • Wait for a callback on the next business day
